No patient identity stored. Care Circles use anonymous codes the database never holds names, ID numbers, or addresses. Notes can be encrypted with your passphrase.

How Care Notes Works

Create a Circle

Complete CareConnect training, then set a passphrase to start your Care Circle. You will get a unique code (e.g. CC-4821). As an exception, you can request access from the coordinator above.

Share with Your Team

Give the code and passphrase to family members and your visiting nurse. They join with no account needed.

Log Care Together

Everyone logs turns, medication, vitals, and tasks in one place. The visiting nurse sees everything since the last visit.
